Note that she is the only provider for this service in our clinics at this time. So, it is important that you verify her schedule before coming in.

Who Qualifies For A Medical Marijuana Certification?

There are 12 medical conditions that qualify for issuance of a Medical Marijuana Certification (329 Card).

  1. Amyotrophic Lateral Sclerosis
  2. Cancer
  3. Glaucoma
  4. Lupus
  5. Epilepsy
  6. Multiple Sclerosis
  7. Rheumatoid Arthritis
  8. Positive status for human immunodeficiency virus
  9. Acquired immune deficiency syndrome
  10. Post-traumatic stress disorder, or
  11. “The treatment of these conditions”, or
  12. “A chronic or debilitating disease or medical condition or its treatment.”

See the Department of Health’s website for specific information on which conditions qualify.

Requirements for the Application:

  • A valid ID.
  • Proof of name, current address and phone number.
  • Current medical records that document the existence of one of the conditions for which medicinal marijuana is authorized under Hawaii state law.
    • Including any relevant x-rays or MRI reports (not the actual films).
  • We cannot act as a patient’s primary doctor, we require that you have at least one other current treating physician to document your condition.
  • New patients MUST provide documentation at their first appointment.

 

WHO QUALIFIES

Under Hawaii state law, several medical conditions may qualify for a medicinal marijuana 329 Card. See the Department of Health’s website for specific information on which conditions qualify.

  • Cachexia
  • IBS
  • Neuropathy

FEES

Health insurance is not accepted for the fees associated with the care coordination and medical services provided for Medical Marijuana 329 Card.

STANDARD SERVICE

  • New Patients who do their application online at home: $200 
  • This does not include Mandatory Department of Health Fee: $38.50
  • Card is valid for 1 year.
  • Renewals – applicants who do their renewal online: $175

CONCIERGE SERVICE

For those who have difficulty with an online process, we can help you through all of the steps. This includes assistance with the application at the time of your visit, as well as uploading your ID & filing the application for you. This DOES NOT INCLUDE the $38.50 DOH fee

  • Concierge Service: $250
  • Mandatory Department of Health Fee: $38.50
  • Card is valid for 1 year.
  • Concierge renewal service: $200
  •  Upload of ID ONLY: $70

NOTE: Cash or Credit Cards only. No checks.

APPLY

  1. Go to https://medmj.ehawaii.gov and click the ‘CREATE FREE ACCOUNT’ link.
    1. Make an ‘egov’ account for yourself, using your email address and your own password. (Write your password down and do not lose it.)
  2. Once your account is set up you will need to sign into https://medmj.ehawaii.gov and fill out the application.
  3. SHANNON RICHARDS is the first and last name of the physician to be filled in at the beginning.
  4. Continue to fill out the application with YOUR information. Double check details as you go. Errors on the application can delay getting your card.
  5. Caregiver information (if applicable) must be current and fully filled out on the appropriate page. Also, you must have the signed papers and IDs for both you and your caregiver (if applicable). (Papers can be grabbed from the ‘Get it here’ button on the upload documents page, three pages total.) READ EVERYTHING CAREFULLY.
    1. If you cannot attach signed papers you will need to bring them to your appointment.
    2. We need both ID’s at the appointment.

  7. Check off grow site box or opt to not have a grow site.
  8. Upload a picture of your ID. If you cannot attach the ID then you must bring it to your appointment.
  9. Scroll to bottom of the last page and select the type of card for the payment then read through and e-sign the ‘Terms and Agreements’
  10. Pay the annual $38.50 application fee to the Department of Health using a credit/debit card.
  11. Write down your APPLICATION NUMBER and BRING IT WITH YOU!

Tips for Applications:

  • Your name MUST MATCH YOUR ID EXACTLY.
  • The ID number is your Driver License, State ID or Passport number. Make sure that matches EXACTLY.
    • All Hawaii driver license start with the letter H followed by ALL NUMBERS. Not the letter “O” after the H.
    • Make sure the ID number matches exactly, if it is from out of state.
  • The Expiration Date field on the application is your ID’s expiration date.
    • Any TEMPORARY ID being used must have the temporary exportation date.
  • SUFFIX BOX is for Jr, Sr, or III (only if on ID) NOT for Mrs. / Mr. / Dr. etc…
  • Your address must contain Rd, St, Hwy, Ln, Blvd, etc.
  • Include Residential address for ‘residential’ and Mailing address for ‘mailing’ (if different).
  • Check off the correct island under residential address. **MAUI**

Bring to your first appointment the following items: 

  • Government issued photo identification from any state (Driver’s License, State ID, US Passport, etc.). We will accept out-of-state identification as long as the presenting patient can provide both a local physical AND mailing address (Proof may be a utility bill).
  • Full payment. We do NOT accept checks.
  • Medical records, medication bottles, or prescription printouts from a pharmacy.
  • Completed paperwork, if printed out at home.
  • Application number
  • Caretakers ID

SCHEDULE

We certify qualified Hawaii residents for Medical Use of Marijuana (MUM). We are not a dispensary and we do not distribute any medical cannabis.

CANNABIS CARD RENEWAL

Do you already have your Hawaii Medical Cannabis Card?  We’re here to help with the renewal of your card. The fee is $175. This does not include the Mandatory DOH fee of $38.50.

Renewal Applications Can Complete Up To 60 Days Before Expiration.

COMPLETE BEFORE BOOKING YOUR APPOINTMENT

Please READ or PRINT instructions THEN click the link to the DOH

  1. Sign into https://medmj.ehawaii.gov with your email address and password.  (The one you used from the prior year.)
    1. Be forewarned that they will prompt you to change your password. This can be as simple as changing the special character or a number. OR feel free to create a whole new password.
    2. If you do not have your password from the year before you will need to click RETRIEVE PASSWORD link to get a temporary one sent to you email.
  2. Once you are signed in to your account click ‘Application Status’ and look for the RENEW button.
    1. Press the RENEW button. It should auto-fill your information from the previous year.
    2. Update any info that needs to be updated, such as your address, ID expiration date, grow-site, etc.
    3. We can help you attach your ID at the office.
    4. If you have a caregiver make sure to update any of their info along with getting the print out pages (caregiver certificate and grow site certificate, 3 total) to have them sign. MAKE SURE TO READ THROUGH AND CHECK OFF THE APPROPRIATE BOXES.
      • You can grab them, to print, on the Upload Documents page by pressing the box that says, ‘Get it here’.
      • Either attach them yourself or bring the signed pages to your appointment along with both valid IDs.
  3. Proceed to the end of the application, sign the Terms and Agreements then pay the annual $38.50 application fee, to the Department of Health, using a credit/debit card.
  4. Make sure you write down your APPLICATION NUMBER you will need this at your appointment.
  5. Come in to renew.

NOTE: If you have a NEW caregiver, we will need their ID.
